I acknowledge and affirm that I/my child am not afflicted with the flu, RSV, Covid, nor any other communicable disease which I can spread to others in this facility.

I acknowledge and accept that participation in group activities comes with inherent risks which could include illness, injury, or loss of property or life, and hold MUCAI Academy harmless for any injury, illness, or loss to me or my child while participating in MUCAI Academy group activities.

I acknowledge and affirm that recreational drugs and weapons of any kind are prohibited inside of MUCAI Academy and at it's events and venues. I will not enter the premises under the influence of recreational drugs, alcohol, or with weapons of any kind.
